INSTRUCTIONS FOR PRESENTATION IN THE VIDEO SESSION

Eight minutes are allocated for your video and seven minutes for questions. The moderator has been instructed to enforce strict adherence to the time allocations. Please give careful attention to the timing of your presentation.

To ensure the proper equipment is in place for your video, please complete the enclosed audio/visual form and return it via fax to 352-331-4975 by May 26, 2006.

You are invited to submit your video to the official Journal of the ASBS, Surgery for Obesity and Related Diseases, to be included in the on-line video system. SOARD is one of the few journals that permit subscribers to access on-line videos. This requires that the video be accompanied by either a full manuscript or a short case report. All videos MUST be eight minutes or less. Videos (on DVD) and manuscript must be submitted to Dr. Harvey Sugerman, Editor, at 290 Southwinds Drive, Sanibel, FL 33957, for review.
